First: does your letter come with a barcode?

Disability requests are the exception that matters. When SSA or a state Disability Determination Services office sends a request with a barcoded cover letter, that barcode page must be the first page of your fax — it is what files your response against the claim automatically. Putting any other cover sheet in front of it breaks the routing and delays the case.

So: barcode letter first, then your documents. Use the cover sheet below only where no barcode page was provided, or behind the barcode page as a summary. Where the letter names an upload route instead, use that.

What belongs on a fax to Social Security

  • The claim, BNC or appeal reference from the letter you received, written the way the letter asks for it. Without any reference your pages have to be matched by hand.
  • Your name exactly as SSA has it, and your date of birth only if the request asks for it.
  • The name of the office or the representative who asked for the documents, if the letter names one.
  • A daytime phone number, the date, and the total page count including the cover sheet.
  • One line saying what is enclosed: "Proof of income, 4 pages, in response to letter dated 12 July 2026".

Which field to use for what

The generator has one set of fields, so put the identifiers where the recipient will look for them:

  • Claim, BNC or appeal reference → Re / subject, alongside the date of the letter you are answering.
  • Any other identifier the letter asks for → Message. Bear in mind an SSA claim number is normally a Social Security number with a suffix, so it is worth asking the office whether a partial reference is enough.
  • The claims specialist named in your letter → To (name); the field office → To (company).
  • What you are sending → Message: "Proof of income, 4 pages".
  • Page count including this sheet → Pages (including cover).

If the letter names an upload through your my Social Security account, that route is faster and gives an immediate receipt.

Keep what you can off the cover page

Faxes to a field office land on a shared machine, so keep bank details and medical information on the pages behind the cover sheet rather than on it. Identifiers are the office's call: write them the way your letter asks.

If the request came from a specific claims specialist, addressing the cover sheet to them by name is the single most useful thing you can do — it is what gets the pages onto the right desk.

Finding the fax number

SSA fax numbers are local to the office or unit handling your case; there is no national fax line for documents, and we do not publish one here.

Use the number on the letter you are answering. If there is none, find your servicing office with the office locator on ssa.gov and call to confirm the fax number and whether they accept the document by fax at all — some requests must go through your my Social Security account or by post.

After you send

  • Keep the transmission confirmation: it evidences the date you responded, which matters when a deadline is attached.
  • Call the office a few working days later to confirm the pages were filed against your claim.
  • Keep your own copy of every page you sent, in the order you sent it.
  • Do not fax the same documents repeatedly — duplicates slow the same queue you are waiting in.

Frequently asked questions

Does SSA accept documents by fax?

Often yes, when the letter you received asks for a fax or names a fax number. Two caveats: disability requests with a barcoded cover letter need that barcode page as page one, and some submissions must go through your my Social Security account or by mail. The letter says which.

What should an SSA fax cover sheet include?

The claim or BNC reference from your letter, your name as SSA holds it, the office or specialist who asked, a daytime phone number, the date, and the page count including the cover.

Is there a national SSA fax number?

Not for documents. Numbers belong to the servicing office or unit, so use the one on your letter or confirm it with your local office through the ssa.gov locator.

Should I include my full Social Security number?

Write identifiers the way your letter asks for them. Where it leaves the choice to you, a partial reference on the cover page and the full details on the forms behind it is the safer split.

Can I fax SSA documents without a fax machine?

Yes. Scan or photograph the pages, download the cover sheet here, and send both from a browser to the office's fax number. If you are a covered entity sending someone else's medical records, use a provider that signs a Business Associate Agreement — FaxBolt does not.
