What belongs on a fax to the VA

  • Your VA file number, exactly as it appears on your VA correspondence. It is the field that attaches the pages to the right claim, so this is not the place to abbreviate unless the letter says you may.
  • Your full name and, if you have it, the last four digits of your SSN as a secondary reference.
  • The claim or appeal the evidence supports, and the form number if you are returning a form such as a 21-4138 statement.
  • A daytime phone number and the date.
  • The total page count including the cover sheet, so an incomplete transmission is visible at their end.
  • One line describing the evidence: "Private treatment records, Dr Reed, January–June 2026, 12 pages".

Medical evidence needs a confidentiality notice

Claim evidence is usually medical, so the sheet should carry a confidentiality notice. The Medical / HIPAA template is already selected above — keep it, and the PHI wording and the CONFIDENTIAL - PHI marking come with it.

Keep diagnoses and clinical detail off the cover page itself. "Private treatment records" is enough; the records behind it carry the specifics.

Finding the right fax number

The VA routes different submissions to different intake points — claims evidence, medical records requests and education or pension paperwork do not share a destination — and the numbers change, so this page does not list any.

Use the fax number printed on the letter you are answering, or the one your VA regional office, VSO or accredited representative gives you. Where the letter offers an upload through VA.gov, that route is usually faster and gives an immediate receipt.

Which field to use for what

The generator has one set of fields, so put the identifiers where the recipient will look for them:

  • VA file number → Re / subject, as written on your letter, together with what the evidence is for.
  • Your name → From (name); the regional office or intake centre → To (company).
  • The representative or VSO who asked → To (name), where the letter names one.
  • What the evidence is → Message: "Private treatment records, Dr Reed, January–June 2026".
  • Page count including this sheet → Pages (including cover).

Keep clinical detail out of these fields — the records behind the cover sheet carry it.

Keep your evidence of sending

  • Save the transmission confirmation with the date. It proves the pages reached that fax number; it does not prove the evidence was filed against your claim, so follow up on the filing itself.
  • Keep a copy of every page in the order it was sent.
  • Follow up with your representative rather than re-faxing; duplicates land in the same queue and slow it down.
  • If the fax fails repeatedly, the number may have been retired — confirm it before assuming the pages arrived.

Frequently asked questions

Does the VA accept claim evidence by fax?

For many submissions, yes, when the letter you received names a fax number. Where VA.gov offers an upload for that form, uploading is faster and gives you an immediate receipt.

What should a VA fax cover sheet include?

Your VA file number, your name, the claim or form the pages support, a daytime phone number, the date, and the total page count including the cover sheet.

Is a VA file number the same as my Social Security number?

For most veterans today the file number is the SSN, but older claims use a separate number. Use whichever appears on your VA correspondence, in full — intake needs it to match the evidence to your claim. If putting it on an open page concerns you, the upload route on VA.gov avoids the cover sheet entirely.

Should the cover sheet have a HIPAA notice?

If the fax carries medical records, yes. Choose the Medical / HIPAA template above and the PHI wording is added. It does not make a transmission compliant on its own, and FaxBolt does not sign a Business Associate Agreement — clinics and other covered entities should send PHI through a provider that does.

Can I fax the VA without a fax machine?
